AN ACT Relating to paid family leave premium collection for dockworkers;
1 SSBSB 5191 (3) "Commissioner" means the commissioner of the department or the commissioner's designee.
2 SSBSB 5191 (i) The service is localized in this state;
3 SSBSB 5191 individual is conducting that is eligible for a business deduction for federal income tax purposes;
4 SSBSB 5191 expenses with the internal revenue service for the type of business the individual is conducting;
5 SSBSB 5191 (ii) Would have qualified for family leave under (b) of this subsection.
6 SSBSB 5191 the last four completed calendar quarters immediately preceding the application for leave.
7 SSBSB 5191 (I) Treatment two or more times, within thirty days of the first day of incapacity, unless extenuating circumstances exist, by a health care provider, by a nurse or physician's assistant under direct supervision of a health care provider, or by a provider of health care services, such as a physical therapist, under orders of, or on referral by, a health care provider;
8 SSBSB 5191 (c) Whether additional treatment visits or a regimen of continuing treatment is necessary within the thirty-day period shall be determined by the health care provider.
9 SSBSB 5191 for treatment for substance abuse by a health care provider or by a licensed substance abuse treatment provider.
10 SSBSB 5191 (a) For an hourly employee, the average number of hours worked per week by an employee within the qualifying period;
