Summary

Adds multiple discharge planning requirements including, (1) policies and procedures addressing high utilizers, (2) personalized crisis planning, (3) engagement of family members, (4) review of discharge instructions with family, (5) assistance with living arrangements and transportation, (6) updated notification requirements for state mental health facility discharges, (7) medication continuity, including clinically appropriate long-acting injectables, (8) care coordination for individuals with intellectual/developmental disabilities, including APD and AHCA licensed facilities, (9) contingency plans when the appropriate level of care is unavailable at discharge, and (10) amends forms incorporated by reference.
