(a) Medicaid reimbursement is available for outpatient mental health services provided by: (b) Outpatient mental health services rendered by a medical doctor, a doctor of osteopathy, or an HSPP are subject to the following limitations: Sec. 1. Medicaid reimbursement is available for emergency and nonemergency transportation, subject to the following restrictions: Sec. 3. Medicaid reimbursement is not available for the following transportation services: Sec. 4. Prior authorization is required for the following transportation services: Sec. 3. (a) Coverage of DSMT is limited to sixteen (16) units of DSMT per for each member per rolling each calendar year without prior authorization. Additional units of DSMT may be authorized via in accordance with the prior authorization process as set forth in 405 IAC 5-3. (b) Coverage of DSMT is limited to the following clinical circumstances:
