Plain-language change summary

The amendments to Bill AB 344 change the terminology from "vulnerable persons" to specifically "older persons and persons with disabilities." This shift clarifies that the bill is focused on these specific groups when discussing their discharge from hospitals and the support they need. By ensuring that hospital staff collaborate with caregivers and coordinate healthcare and social services, the changes aim to improve the transition experience for these individuals, which is crucial for their recovery and overall wellbeing.
