This bill mandates that parents or guardians of children enrolling in public schools acknowledge the importance of medical and dental physicals by signing a statement.

  • Section 37-15-9

(4)(b) When any child applies for admission or enrollment in any public school in the state, the child's parent, legal guardian or legal custodian, shall acknowledge receipt of notice on the school registration form of the importance that his or her child receives a medical and dental physical. The registration form shall include the following statement with a box to be checked by the parent, legal guardian or legal custodian acknowledging that notice has been provided by the school: "By checking this box, I am acknowledging I have received notice that it is beneficial to the overall health of my child and is important that my child receive a medical and dental physical prior to the first day of school if he or she has not already received one." The parent, legal guardian or legal custodian of the child shall be required to check the box and sign the accompanying acknowledgment at the start of each academic school year. If a child transfers between school districts and it is determined from the child's cumulative record or application for admission or enrollment that the child's parent, legal guardian or legal custodian has failed to sign the acknowledgment at the time of enrolling the child in the previous school district, the new school of enrollment shall communicate notice of the required acknowledgment along with the statement of acknowledgment to the parent, legal guardian or legal custodian and have him or her check the box and sign the accompanying acknowledgment as soon as possible.

This adds a requirement for parents or guardians to acknowledge the need for children's medical and dental physicals when enrolling in school.

  • Section 37-15-11

The child's parent, legal guardian or legal custodian shall comply with the requirement for an acknowledgment of receipt of notice of importance that the child receives a medical and dental physical prescribed in Section 37-15-9(4).

This change incorporates the new acknowledgment requirement concerning physicals into the enrollment process.
