1 SHBHB 1162 to or prevent the risk of violence, including but not limited to the following:
2 SHBHB 1162 NEW SECTION.
(3)(a)(3) The health care setting must submit a report to the committee identified under RCW 49.19.020(1)(b) aat summaryleast ofquarterly, unless the following:health care setting is a critical access hospital under 42 U.S.C.
(i) The data required by RCW 49.19.040 and the findings of investigations required by this section during the relevant time period, with any personal information deidentified in compliance with the federal and state law;
(ii) An analysis of any systemic and common causes of the workplace violence incidents;
and (iii) Any relevant recommendations for modifying the plan under RCW 49.19.020 and other practices in order to prevent future incidents of workplace violence.
(b)(i) The summary must be submitted at least twice per year for any of the following health care settings:
(A) A critical access hospital under 42 U.S.C.
1395i-4;1395i-4, in which case it must submit a report at least twice per year, with the following:
(B)(a) A hospitalsummary withof fewerthe thandata 25required acuteby careRCW beds49.19.040 and the findings of investigations required by subsection (1) of this section during the relevant time period, with any personal information deidentified in operation;compliance with the federal health insurance portability and accountability act of 1996;
(C)(b) A hospitalsummary certifiedand byanalysis theof centersany forsystemic medicare and medicaidcommon servicescauses asof athe soleworkplace communityviolence hospitalincidents; that is not owned or operated by a health system that owns or operates more than one acute hospital licensed under chapter 70.41 RCW;
orand (D)(c) ARecommendations hospitalfor locatedmodifying onthe anplan islandunder operatingRCW within49.19.020 aand publicother hospitalpractices district in Skagitorder county.to prevent future incidents of workplace violence.
(ii) The summary must be submitted at least quarterly for all other health care settings.
(4) This section does not affect or supersede any other state or federal law that prohibits or limits the disclosure of personally identifiable information.
