A HEALTH CARE PROVIDER SHALL INDIVIDUALLY REVIEW EACH CLAIM OR PRIOR AUTHORIZATION THAT INVOLVES MEDICAL NECESSITY, EXPERIMENTAL STATUS OR THAT REQUIRES THE USE OF MEDICAL JUDGMENT BEFORE A HEALTH CARE INSURER MAY DENY A CLAIM OR A PRIOR AUTHORIZATION.
