ON THE REQUEST OF A PARENT OR GUARDIAN, EACH SCHOOL SHALL PROVIDE INFORMATION ABOUT THE TRAINING PROGRAM AND AVAILABLE RESOURCES DESCRIBED IN SUBSECTION A OF THIS SECTION TO THE PARENT OR GUARDIAN OF A STUDENT AT THE SCHOOL.
INFORMATION PROVIDED PURSUANT TO THIS PARAGRAPH SHALL INCLUDE INFORMATION ABOUT HOW PARENTS AND GUARDIANS MAY ACCESS TRAINING IN ALTERNATIVE FORMATS THROUGH PUBLIC SCHOOLS, INCLUDING CONTACT INFORMATION.] B.
THE INFORMATION PROVIDED TO THE PARENT OR GUARDIAN SHALL INCLUDE ALL OF THE FOLLOWING:
