Amend House Bill 4698 by replacing everything after the enacting clause with the following:
"Section 5.
"During a preventive or wellness visit, if acute complaints or chronic problems which require additional evaluation are discussed, you may be billed for multiple services, including a separate office visit charge, billing for diagnostic testing or follow-up, or both.
