anDand WHEREAS,E originallycurrent knownname as Faith in Action,2020; Wheels for Wellness was created as a result of a community roundtable and established through a $35,000 grant from the Robert Wood Johnson U Foundation;

andthe Corganization currentserves nameindividuals in 2020;the City of Winchester and the Counties of Clarke, Frederick, Shenandoah, and Warren;

andandot WHEREAS, Wheels for Wellness provides transportation for individuals who include those who do not qualify for social service programs, can no longer safely drive themselves, cannot afford to own a vehicle,WHEREAS, orbetween do2016 notand have2021, aWheels familyfor memberWellness orcompleted friend14,738 transports to assistmedical withtraveled; transportation;

and appointments, radiation and chemotherapy treatments, and dialysis treatments, representing 323,558 miles traveled;WHEREAS, Wheels for Wellness has fulfilled its mission through the hard work of its executive R director and scheduling manager and the dedication and generosity of its volunteer drivers, who use t3eir itn vehicles and gas to provide transportation for their fellow community members;
